What Does Medicare Cover for Home Health Care?

Understanding Medicare coverage for home health care can be confusing. This guide breaks down exactly what is covered, who qualifies, and how to access these benefits.

Medicare Home Health Coverage at a Glance

Medicare covers home health care services at 100% with no copay or deductible when you meet the eligibility requirements. This is one of the most generous benefits in the Medicare program, yet many eligible patients don't take advantage of it because they don't know it exists.

What Medicare Covers

Part-time or Intermittent Skilled Nursing

Medicare covers skilled nursing services on a part-time or intermittent basis. This means up to 8 hours per day and 28 hours per week, with some exceptions allowing up to 35 hours per week for limited periods.

Physical Therapy

In-home physical therapy is covered when ordered by a physician as part of your care plan. There is no limit on the number of visits as long as they are medically necessary.

Occupational Therapy

Similar to physical therapy, occupational therapy is covered when medically necessary and ordered by your doctor.

Speech-Language Pathology

Speech therapy services are covered to address communication, swallowing, and cognitive disorders.

Medical Social Services

Medical social work services including counseling, care coordination, and resource connection are covered.

Home Health Aide Services

Personal care services from a home health aide are covered, but only when you are also receiving skilled nursing or therapy services.

What Medicare Does NOT Cover

  • 24-hour home care
  • Meal delivery services like Meals on Wheels
  • Homemaker services (shopping, cleaning, laundry) when this is the only care needed
  • Personal care (bathing, dressing) when this is the only care needed
  • Custodial care that does not require skilled medical professionals

  • You must be under the care of a doctor who creates your care plan
  • You must need and receive at least one of: skilled nursing, physical therapy, speech therapy, or continued occupational therapy
  • You must be homebound
  • The home health agency caring for you must be Medicare-certified
